Staying Safe in Ipoh

Ipoh is a quiet, low-crime provincial city and is one of the more relaxed places in Malaysia to walk around. The real risks are ordinary and environmental: heat, which is worse here than on the coast because the valley traps it; traffic, which does not defer to pedestrians; and, for anyone doing the caves or the rivers around Gopeng, the specific hazards of caving and white water. None of it is unusual and all of it is manageable.
Heat and the valley
Ipoh is inland, enclosed by hills and noticeably hotter and stiller than Penang or Kuala Lumpur — 32–34°C with little breeze. Heat exhaustion while walking the old town in the middle of the day is the single most likely thing to spoil a visit. Walk before ten and after four, use the covered five-foot ways, drink far more than feels necessary and wear a hat. The cave temple climbs, particularly Perak Tong's several hundred steps, are best done early.
Traffic and crime
The old town's streets are narrow, one-way systems are confusing and drivers do not stop for pedestrians as a default; motorcycles use gaps and pavements. Look both ways on one-way streets. Crime is low, but snatch theft from passing motorcycles — the characteristic Malaysian street crime — occurs; carry bags on the side away from the road with a cross-body strap. Ipoh is comfortable to walk at night in the old town and New Town.
Caves and rivers
Gua Tempurung's longer tours involve wading, crawling and swimming through an underground river in the dark; they are run with guides for good reason and should not be attempted independently. Water levels rise fast after heavy rain and tours are suspended, which is a decision to respect. White-water rafting on the Kampar river at Gopeng is grade II to III and safe with a reputable operator — ask about guide certification and safety kayaks. Wear footwear that stays on your feet for both.
Health and practicalities
Hawker food in Ipoh is safe and hygiene grades are displayed. Tap water is treated but bottled is standard. Dengue is present across Malaysia year-round; use repellent. Ipoh has good private hospitals and is a minor medical-tourism destination. Emergency number is 999. Malaysia's drug laws carry mandatory death-penalty provisions for trafficking and are enforced. And modest dress is appreciated at the cave temples and mosques, with shoes removed where indicated.
